Please complete this form if you are a parent/guardian of an applicant for the PAYA Youth Council and your child is under the age of 18 as of June 1, 2026. The deadline for submission is Thursday, April 23 at 11:59 pm Pacific time.

If your child will be 18 or older as of June 1, you do not need to complete this form.
